2、残疾人证遗失，应及时报告户口所在地县级残联，声明作废后可申请补发。补发残疾人证编号在原20位编号后加印“B”，第二次遗失补发加印“B2”，以此类推。同时，遗失的残疾人证在残疾人人口数据库中标明作废。

3、残疾人证污、损，影响正常使用，可以将污、损残疾人证交回户口所在地县级残联作废并免费换领。换领残疾人证登记信息与原残疾人证一致。

4、残疾人户口迁移，需到原户口所在地县级残联凭公安机关出具的户口迁移证明开具残疾人证迁移证明。残疾人凭原户口所在地县级残联转出的残疾 人证申请表、评定表等档案材料和出具的残疾人证迁移证明，到新户口所在地县级残联登记入档。新户口所在地县级残联在残疾人证备注栏中注明迁移日期并加盖公 章，留存转来的材料档案，并将复印件上报市级残联归档。

5、残疾人康复脱残或死亡的，发证机关应及时将残疾人证收回。残疾人康复脱残以县级残联指定的县级（含县级）以上医院或专门医疗机构残疾评定结果为准。
